Thank you for scheduling your pet(s) for the HATS Spay Neuter transport!
The following form will need to be filled out prior to transport. If you have a printer, please print it, fill it out and bring it with you the morning of the transport. Blank forms will be available the morning of transport but the more forms we have filled out ahead of time the more quickly we can get your pets on the road!
In addition to the form, you will need:
- Proof of rabies and distemper vaccines. If your animal is not current on vaccines, please be prepared to pay $11 for each required vaccine.
- Recent bloodwork results if your pet is 7 years old or older. (You are responsible for scheduling your pet for bloodwork at your own vet.)
- A plastic, hard-sided carrier appropriate for your pet’s size. If, for example, you have a large dog crate for a cat, you will need to rent a crate sized for a cat from us that morning.
- E-collars are a good idea for pets who have surgery. We will have a few on hand that you can purchase if you need one. You can also purchase them at your vet’s office or some pet supply stores.
